​A credit card will be requested upon intake. We ask that you inform us of any changes (expiration, cancelation) so that an active card is on file at all times.​​​​​​

If your clinician is a fully licensed clinician (LPC, LCSW)

If your clinician is a provisionally licensed master’s level clinician (PLPC, LMSW)

If your clinician is a master’s level intern

*Sliding Scale: NOGC offers services from $30 - $100 for those who are court appointed or in need of financial assistance*
